Job summary

Leaders in Care, Gloucester, is recruiting an experienced Care Coordinator to join a large homecare organisation. This Monday to Friday role combines office-based coordination with some field duties, and you will need a full UK driving licence and access to your own vehicle.

You will coordinate rotas, support service delivery and help ensure care plans meet each client’s needs, while building strong relationships with care workers, clients and families.

Responsibilities

• Coordinate care schedules and maintain rotas.
• Support allocation of care workers across community packages.
• Build relationships with care staff, clients and families.
• Support assessments and reviews of individual care plans.
• Supervise and support care workers in the community.
• Respond to changes in care requirements and staffing needs.
• Ensure records and systems are kept accurate and up to date.
• Support the wider branch team to maintain high-quality service delivery.
• Participate in the on-call rota.
