Job summary

City of Doncaster Council is recruiting for a Community Care Officer (CDC) within the Mental Health Social Work Team. This permanent, full-time role focuses on early intervention, referral triage and direct work to maximise independence and wellbeing.

The post offers 37 hours per week, Grade 7 salary (£29,542 - £32,578), and flexible/hybrid working arrangements where possible, with a commitment to safeguarding and high-quality social care.

Responsibilities

  • Promoting independence and reducing risks for adults living in Doncaster in line with the Care Act (2014)
  • Undertaking a person-centred care act assessment to identify need taking the appropriate action whilst respecting individual values, cultures and diversity
  • Identifying needs and creating support plans to meet identified needs as well as determining eligibility for services in accordance with Care Act Guidance
  • Management of a caseload acknowledging that some individuals may require assistance from other professionals
  • Identification of risks or incidents of possible abuse including reporting this to the appropriate staff member in line with the Councils Safeguarding adults policy and procedure
  • Supporting and working with Qualified Social Workers and other professionals in cases which have become complex
  • Assisting in the development and implementation of the locality-based community led support model using creative ways to support service users and carers

There is an exciting opportunity to join the Mental Health Social Work Team in the role of Community Care Officer. This exciting pilot opportunity within the Mental Health Social Work Team will focus on supporting early intervention and prevention. The Community Care Officer will assist with referral triage, undertaking direct work with individuals to help maximise independence and wellbeing, undertake assessments where required, and develop strong working relationships with community and voluntary sector partners. The role offers the opportunity to make a meaningful difference by ensuring people receive the right support at the right time.

The Mental Health Social Work Team work in partnership with secondary mental health services to provide an integrated, high-quality Social Work service to adults and carers in accordance with the Care Act (2014) statutory requirements, local policy, procedure and guidance. We work with a diverse population including preparing young people for adulthood, working age adults and older people with functional mental health needs.

Key Responsibilities:
