Job summary

City of Doncaster Council is seeking a STEPS Community Reablement Officer to join the team. Based at Holmescarr Centre, Rossington, you will work 25 hours per week on a 14‑day rota, evenings or mornings, on a permanent contract.

You will support people in their own homes for up to six weeks after hospital stays, helping maintain independence with practical support, personal care, mobility transfers, meals, safeguarding and risk assessments, with enhanced DBS required.

  • Experience in social care settings and ability to work in a reablement context.
  • Ability to work evenings or mornings on a 14-day rota.

STEPS team (Short Term Enablement Programme) need a Community Reablement Officer that can demonstrate the 6 C’s.

Care –Compassion – Competence – Communication – Courage – Commitment

You will support people in their own home for up to 6 weeks following a hospital stay or to prevent a hospital admission. You will assist and support people to live how they choose in a safe environment while promoting independence with practical support as agreed in their support plan; this is called reablement.

Responsibilities will include providing personal care, assisting people with mobility transfers following a movement & handling plan, providing support with medication, meals and understand safeguarding and risk assessments.

STEPS are regulated by the Care Quality Commission; you will understand how ways of working in the team makes sure we maintain the highest standard of care and excellence.
