The Community Care Officer will assist with referral triage, undertaking direct work with individuals to help maximise independence and wellbeing, undertake assessments where required, and develop strong working relationships with community and voluntary sector partners. The Mental Health Social Work Team work in partnership with secondary mental health services to provide an integrated, high-quality Social Work service to adults and carers in accordance with the Care Act (2014) statutory requirements, local policy, procedure and guidance. We work with a diverse population including preparing young people for adulthood, working age adults and older people with functional mental health needs.
