Job summary

Manchester University NHS Foundation Trust (MFT) invites applications for a Care Navigator within the Integrated Neighbourhood Teams on the Oxford Road Campus. The role links hospital and community services to support safe discharge, signpost patients, and coordinate care across health, social care and VCSE partners.

You will work with multidisciplinary teams, manage information sensitively, and help identify patient needs early.

Qualifications

• A-level / NVQ 3 or equivalent experience in admin / business / marketing / customer service.
• Experience with IT solutions and with patients in a clinical environment.
• Ability to meet deadlines.
• Experience in multidisciplinary teams.
• Experience handling sensitive or confidential information.
• Proven administrative experience.
• Leadership and development qualities; experience supervising junior staff.
• Experience undertaking Appraisals.

Responsibilities

• Coordinate care navigation across hospital and community services.
• Signpost and link people to relevant services in the community.
• Support discharge planning and reduce unnecessary admissions.
• Work with Neighbourhood Multi-agency Meetings and MDTs.
• Promote patient access and maintain follow-up in the community.
