OneCall24 Healthcare in Chelmsford is seeking a Community Care Lead to provide operational leadership to care teams and oversee client care packages, ensuring services are delivered safely and to a high standard.

You will collaborate with clinical, scheduling and compliance teams, develop staff, monitor quality, and build strong relationships with clients and commissioners while traveling as required.

Formación

• Minimum 1 year's experience in care coordination and a senior or supervisory role.
• Experience in complex care, supported living, or similar environments.
• Knowledge of safeguarding, regulatory standards, and care delivery.

About the Role

As a Community Care Lead, you will provide operational leadership to care teams, oversee client care packages, and ensure services are delivered safely, effectively, and in line with regulatory standards. Working closely with operational, clinical, recruitment, compliance, and scheduling teams, you will play a key role in maintaining high-quality, person-centred care.

Responsibilities

• Lead, support, and develop care staff through supervision, appraisals, and ongoing guidance.
• Oversee client care plans, assessments, quality reviews, and service delivery.
• Support workforce planning, rota management, and staff engagement.
• Promote safeguarding, compliance, and best practice across the service.

Requirements

• Minimum 1 year's experience in care coordination and a senior or supervisory care role.
• Previous experience working within complex care, supported living, or a similar environment.
• Strong knowledge of care delivery, rota planning, safeguarding, and regulatory standards.
• Excellent communication, organisation, and people management skills.
• Flexible, proactive, and willing to travel as required.
