Job summary

Nottinghamshire Healthcare NHS Foundation Trust's community nursing team is seeking a suitably skilled nurse to join our physical healthcare services for the resident adult population of Sherwood. You will provide holistic community care in patients' homes and clinics, and collaborate with a multi-disciplinary team to support patient independence.

The role requires excellent communication, adaptability and reliability, with a robust induction and preceptorship for newly qualified staff.

Qualifications

• Qualified nurse with current NMC registration and degree in nursing.
• Experience or ability to work in community/home settings.
• Able to work flexibly in a fast-moving service.

Responsibilities

• Provide skilled nursing care in residents' homes and clinic settings.
• Work with MDTs to support patient journey and prevent hospital admissions.
• Maintain high standards of assessment, intervention and evaluation.
• Communicate effectively with patients, families and colleagues.
