Job summary

The Newcastle upon Tyne Hospitals NHS Foundation Trust is recruiting a Community Cardiac Rehabilitation Worker to support home-based exercise programmes for patients post-MI, after PCI or cardiac surgery, and for heart failure. The role involves visiting patients in their homes and supporting the band 7 physiotherapist in programme delivery.

The post requires driving and business insurance, with supervision provided. 37.5 hours per week and a closing date of 16 November 2026.

Qualifications

• Demonstrates basic IT literacy for patient records and communications.
• Ability to interpret basic clinical measurements and observations.
• Requires NVQ3 or equivalent with demonstrated experience at HCA level 3.
• BACPR or equivalent training to support cardiac rehab programmes.

Responsibilities

• Assist nurses and physiotherapists in delivering care in the community.
• Check baseline observations before delivering prescribed exercise plans.
• Cover cardiac rehabilitation group classes as needed across multiple centres in the city.
• Provide home-based exercise instruction and monitoring.
• Ensure patient privacy and dignity at all times.
• Work with the physiotherapist and nursing staff to deliver home exercise programmes.
• Contribute to the cardio rehab multidisciplinary team.
