Center for Life Management (CLM) is hiring an Integrated Care Medication Training and Support Specialist to provide outpatient, community-based counseling and education on medication management. This full-time position focuses on improving adherence and health outcomes for adults with complex or chronic conditions.

Qualificações

• Bachelor’s degree preferred in nursing, psychology, social work, or counseling; Associate’s degree may qualify.
• Ability to exercise sound clinical judgment in routine, urgent, and emergent situations.
• Proficient with Microsoft Office applications.
• Ability to document effectively during clinical work.
• Ability to navigate an electronic health record.
• Work as part of a clinical team while remaining self-directed.
• Willingness to acquire new clinical skills as needed.
• Effective communication and strong organizational skills.
• Ability to multi-task in a fast-paced environment.

Responsabilidades

• Provide counseling and Functional Support Services in the home or community.
• Deliver Individual Therapeutic Behavioral Services to reduce symptoms.
• Provide Family Training and Counseling to support resiliency.
• Provide Medication Training and Support to incorporate prescribed medications into routines.
• Provide Crisis Intervention when symptoms worsen and ensure safety.
• Collaborate with treatment team and external providers for continuity of care.
• Maintain billable hours and eligibility for incentive pay.
• Represent CLM professionally in all settings.
• Complete required documentation per agency policies and standards.
