Job summary

Milestone LLC seeks a Support Specialist to deliver training, support and AOL's services within the community setting. You will handle client documentation, reports and develop treatment goals with service coordination.

You must maintain a mobile phone and DMV license, with a monthly vehicle stipend. The role reports to the Clinical Supervisor and involves liaising between family, referral sources and community organizations, coordinating services, and ensuring confidentiality in all client

Qualifications

• Bachelor's degree with 1 year related human services experience or 3 years with unrelated degree + 15 credits in Human Services
• Experience in case management and service planning
• Crisis intervention and report writing

Responsibilities

• Participate in the service plan within the initial thirty-day period
• Monitor length of client services and notify referral sources before ninety-day completion
• Maintain case records and document services rendered
• Attend staff meetings and coordinate staffing with management
• Maintain confidentiality and liaison with families, referrals, and agencies
• Provide crisis intervention as needed
