Job summary

UCS College Group is seeking a School Transitions Officer to work with local schools and external agencies to support young people in making a successful transition into college. The role focuses on recruitment, progression and retention, with a specific emphasis on primary schools in Sedgemoor and cross-college responsibilities.

The postholder will attend EHCP annual reviews, provide guidance to learners with additional needs, arrange taster sessions and help develop learner-centred transition

Responsibilities

• Build and maintain relationships with local schools and external agencies
• Coordinate structured and supportive transition pathways for learners with SEND needs and EHCPs
• Attend planning meetings and EHCP annual reviews to understand potential student needs and guide course selection
• Arrange taster sessions and develop learner-centred transition plans
• Contribute to EHCP consultation processes and provide responsive support to enable positive outcomes
