Job summary

Norfolk Community Health and Care NHS Trust is seeking a Clinical Support Worker to assist in delivering high-quality patient care under the supervision of registered staff within community health settings.

You will follow the Approved Core Task List, maintain patient confidentiality, document care, support admissions and discharges, and help keep environments safe and clean. The role requires empathy, clear communication, and the ability to work with MDTs in emergencies.

Responsibilities

• Accurately and promptly carry out instructions of the Registered Nurse (RN) / Registered Nursing Associate (RNA) who is professionally accountable for the delivery of care.
• Report to registered staff any patient that gives cause for concern or any observed changes in a patient’s clinical condition.
• Support patients' emotional well-being by supporting positive conversations and encouraging participation in ward-based activities.
• Accurately document all care delivered, including electronic recording on trust systems while maintaining confidentiality at all times.
• Respond appropriately to clinical emergencies to ensure the patient receives appropriate care.
• Be aware of and adhere to all trust policies, procedures and nursing guidelines when undertaking any activities.
• Assist in the admission process and make patients welcome to the clinical area under supervision of registered staff.
• Provide and deliver a high standard of individualized nursing care and keep the environment clean and safe.
• Assist in the discharge process for patients directed by a registered staff and MDT.
