Job summary

Frimley Health NHS Foundation Trust is seeking a Band 6 Clinical Practitioner to support All-Age Complex and Continuing Care (AACCC). You will assess complex needs, apply professional judgement, and work with families and professionals to ensure timely, person-centred decisions.

You will undertake CHC assessments and reviews, developing experience across the CHC pathway while collaborating with diverse teams to deliver safe and effective care.

Qualifications

• Experience in NHS Continuing Healthcare (CHC) assessments or a strong interest in CHC pathway.
• Registration as a healthcare professional with relevant practice.
• Ability to work across multidisciplinary teams to support person-centred decisions.

Responsibilities

• Undertake NHS CHC assessments and reviews with individuals and families.
• Collaborate with multidisciplinary teams to understand needs and make evidence-based recommendations.
• Advocate for individuals to ensure safe, appropriate and responsive care.
• Contribute to person-centred planning and care coordination across services.
