We have an exciting opportunity to join our All-Age Complex and Continuing Care (AACCC) service as a Band 6 Clinical Practitioner, supporting children, young people and adults with some of the most complex health and care needs across Thames Valley.

As a Clinical Practitioner, you'll play a vital role in ensuring people receive the right care, in the right place, at the right time. Working closely with individuals, their families and a wide range of health and social care professionals, you'll use your clinical expertise to assess complex needs, support person-centred decisions and help people achieve the best possible quality of life.

It's an exciting time to join us. Having recently become part of Frimley Health NHS Foundation Trust, we're embarking on an ambitious programme of transformation, bringing our teams together, strengthening our clinical practice and developing new ways of working. At the heart of your role will be undertaking NHS Continuing Healthcare (CHC) assessments and reviews, working closely with individuals, their families and multidisciplinary teams to understand their needs and make evidence-based recommendations in line with the National Framework. You'll also have opportunities to develop your experience across the wider CHC pathway, including Fast Track, care planning, clinical commissioning, case management and personal health budgets.

No two days will be quite the same. You'll work with people in a variety of settings, navigate complex clinical situations and bring information to influence different professionals to support person-centred decision making.
