Qualifikationen

• Support patients to get the best outcomes from their medicines.
• Monitor high-risk drugs, particularly renal function for patients on anticoagulants.
• Carry out medication reviews and reconciliation after hospital discharge.
• Visit care homes to review medicines needs and develop action plans.
• Manage the team inbox on rota, handling emails and tasks from PCN and practice staff.
• Receive and process MHRA alerts and advise practices.
• Assist the care home vaccination programme, administering vaccines if qualified.
• Reconcile medicines after patients are discharged from hospital or clinic.
