Job summary

NHS Jobs is seeking a Clinical Pharmacist (IP) for maternity cover to join GP Practices within the Primary Care Network. You will provide expert medicines management, lead structured med reviews, and support safe prescribing across the network.

You will work in a multi-disciplinary team, liaise with GPs and pharmacists, and help reduce hospital admissions through optimized therapies. CPPE pathway completion or willingness, plus prescribing qualifications, is desirable.

Qualifications

• Undergraduate Pharmacy degree and GPhC registration.
• Experience in GP practice or primary care is desirable.
• Excellent communication and ability to work within a multi-disciplinary team.
• Completion or willingness to complete the CPPE pathway; prescribing qualification encouraged.

Responsibilities

• Provide clinical medicines management within GP practices to optimise therapy and cost-effectiveness.
• Undertake holistic structured medication reviews to reduce polypharmacy where appropriate.
• Manage transfers of care and safe prescribing; handle repeat prescriptions and acute requests.
• Contribute to reductions in hospital admissions by supporting optimal medicine use.
• Collaborate with PCN team and community pharmacists to support medicine adherence.
