Qualifications

• Registered with the General Pharmaceutical Council (GPhC).
• VT Foundation Framework or equivalent band 4/5 experience.
• Experience in medicines management not essential; full training will be provided.
• Strong communication, organisational, and teamwork skills.
• Commitment to professional development and service improvement.

Responsibilities

• Support medicines reconciliation and discharge planning.
