Job summary

CNTW NHS Foundation Trust is seeking a registered pharmacy technician to join its mental health services in the North East region. The role covers medicines reconciliation on admission, supporting patients and carers, and medicines optimisation across physical and mental health needs.

You will work with off-site community teams and in Clozapine clinics, providing medicines management, patient reviews and health monitoring. Comprehensive training and clinical supervision will be provided.

Qualifications

• Must be a registered Pharmacy Technician with current registration.

Responsibilities

• Medicines reconciliation and review on admission.
• Provide information to patients and carers to assist with medicine management.
• Optimise medicines with a holistic view of mental and physical health needs.
• Coordinate medicines optimisation with off-site community teams.
• Support Clozapine clinics with medicines management, patient reviews and health monitoring.
• Provide physical health and wellbeing advice to patients and staff.
• Assist discharge planning and liaison with patients and community pharmacies.
• Support dispensaries with accuracy checking slots.
