Job summary

CNTW NHS Foundation Trust is seeking a Clinical Pharmacy Technician to support medicines optimisation and direct patient care across inpatient and community settings in the North East of England.

The post requires GPhC registration and a qualified Pharmacy Technician with post-qualification experience; comprehensive training will be provided.

You will contribute to discharge planning, Clozapine clinics and dispensary support, with opportunities to develop clinical knowledge and skills.

Qualifications

• Registered with the General Pharmaceutical Council (GPhC) is essential.
• Qualified Pharmacy Technician with post-qualification experience is required.
• Accredited checker of dispensed items and NVQ assessor qualifications preferred.
• Must be willing to undertake NVQ standardisation requirements and clinical training.

Responsibilities

• Medicines reconciliation and review on admission.
• Provide information to patients and carers about medicines.
• Optimise medicines with a holistic view of mental and physical health.
• Coordinate medicines optimisation in off-site community teams.
• Support Clozapine clinics with medicines management and patient reviews.
• Provide health and wellbeing advice to patients and staff.
• Discharge planning and liaison with patients, community teams and pharmacies.
• Provide operational support to SNH dispensaries with accuracy checks.
