Lower Broughton Health Centre in Salford seeks an experienced Clinical Pharmacist to join our established team. The role focuses on medicines optimisation, prescribing safety and quality improvement to deliver high standards of patient care.

The post is a full-time position (37.5 hours per week) with a salary of £49,000–£56,000 depending on experience, NHS pension and annual leave. Independent Prescribing qualification and CPPE pathway are valued, with support for professional development.

Formación

• 12+ months experience working in primary care.
• Independent Prescribing qualification (working towards or completed).
• GPhC registration as a pharmacist.

Responsabilidades

• Undertake Structured Medication Reviews (SMRs) to optimise treatment and patient outcomes.
• Identify and reduce inappropriate polypharmacy and medicines waste.
• Reconcile medicines after hospital discharge and liaise with patients and hospital/community teams.
• Address medicines-related problems to reduce avoidable admissions and readmissions.
• Review repeat prescriptions and monitoring requirements to ensure safe prescribing.
• Provide medicines information and training to staff and patients.
