The Pharmacist Network is seeking an onsite IP Clinical Pharmacist to support a partner practice in Salford. You will work within a multidisciplinary team to ensure safe, effective medicines optimisation and patient-centred care.

The role emphasizes clinical leadership and proactive medication management. Requirements include GPhC registration, IP qualification, EMIS/System1 experience, CPPE Primary Care pathway, and the ability to commute to the Salford location.

Qualifikationen

• 12 months experience in general practice/primary care
• GPhC registered
• Independent prescribing qualification
• Experience with EMIS or System 1
• CPPE Primary Care pathway completed or near completion
• Able to commute to onsite location or live nearby
• Excellent communication and interpersonal skills

Aufgaben

• Act within professional boundaries, supporting and working with pharmacists and pharmacy technicians
• Resolve medication-related queries and support multidisciplinary team
• Improve patient outcomes and access to healthcare
• Conduct medicine reconciliation and post-discharge reviews
• Provide expert medication advice to clinicians and patients
• Deliver clinical leadership on medicines optimisation and quality improvement
