Job summary

Mosaic Primary Care Network is recruiting a Clinical Pharmacist to join the Bramblys Grange Medical Practice and the Odiham/Old Basing centres. The role involves medicines optimisation within a multi-disciplinary primary care team, with prescribing responsibilities either current or through an 18-month training pathway.

You will contribute to patient safety and outcomes, undertake structured medication reviews, and support integration with wider health services.

Qualifications

• Experience of working in primary care.
• Experience in a GP practice.
• Strong organisational and IT skills.

Responsibilities

• Work as part of a multi-disciplinary team to clinically assess and treat patients using medicines expertise.
• Be a prescriber or training to become one, within an 18-month pathway.
• Lead clinical medication reviews for patients with polypharmacy, especially elderly and those in care homes.
