Job summary

Bridgwater Bay Primary Care Network is seeking a Clinical Pharmacist to join our multi-professional Primary Care Team. The role involves delivering safe, effective and cost-efficient medicines use, including medicine reviews, and contributing to medicines optimisation across practices.

You will provide clinical support as an Independent Pharmacist Prescriber, delivering face-to-face, telephone and video appointments, with potential Saturday rota involvement as part of extended hours.

Responsibilities

  • Provide clinical support to the surgeries within the Bridgwater Bay Primary Care Network as an Independent Pharmacist Prescriber, consulting with patients through face-to-face clinics, telephone appointments and video consultations.
  • Provide clinical support to patients in care and nursing homes as well as the wider community.
  • Carry out structured medication reviews for all patient cohorts.
  • Support the PCN practices to improve medicines reconciliation on discharge, formulary compliance, repeat prescribing management and medication reviews.
  • Support Bridgwater Bay PCN and Somerset ICB plans to reduce medicines waste in primary care.

Job description

We have an exciting opportunity for a Clinical Pharmacist to join our multi-professional Primary Care Team working within our Health & Wellbeing Hub under the direct supervision of the Clinical Director. If you are a dynamic pharmacist, interested in continuing or developing your career in primary care then we would love to hear from you!

The successful candidate will work to deliver safe, effective, and cost-effective use of medicines by applying their pharmaceutical, clinical and technical skills. This role will provide a brilliant opportunity for the right candidate to develop their knowledge and skills. In addition, they will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments. This will involve assisting the PCN in achieving national requirements, NICE implementation and utilisation of medicines optimisation initiatives.

  • This role is part of the extended hours provision and may include Saturdays on a rota basis.

Bridgwater Primary Care Network (PCN) is the largest PCN in Somerset with 9 GP practices and a diverse population spread across town and rural locations.

As a PCN we are forward thinking, innovate and driven to deliver the best patient care for our population. This includes health population management, and this role ties in with supporting that and tracking the improvements we can make to patients lives.

In partnership with Somerset NHS Foundation Trust, we have developed a joint venture to open a Health and Wellbeing Hub at the former Victoria Park Medical Centre. This flagship hub, the first of its kind, brings together Primary and Secondary Care under one roof to provide joined-up services and support for the Bridgwater Bay community.
