Job summary

Primary Care Doncaster Ltd invites applications for Clinical Pharmacists to join the North Primary Care Network in Doncaster. The post offers multidisciplinary team support, including supervision from an senior pharmacist, and a wide range of pharmacy services across three practices.

You will optimise medicines use, handle prescription queries, enhance prescribing safety, and educate patients on self-management while ensuring appropriate referrals when needed.

Qualifications

• This role requires a GPhC registered pharmacist with an MPharm or equivalent.
• Additional Independent Prescriber qualification is desirable or to be completed.
• Experience working in primary care settings is essential.

Responsibilities

• Provide pharmacy services across three practices within the PCN.
• Carry out chronic disease management and medicines reviews.
• advise on prescription and medication queries to improve safety.
• Support repeat prescribing and medicines reconciliation on transfer of care.
