Job summary

The Pharmacist Network is seeking an IP Clinical Pharmacist to join an onsite practice in Carlisle, full time. You will support day-to-day operations, provide pharmaceutical care, and advise patients while adhering to safety guidelines.

You will work with a team of pharmacists and technicians to resolve medication queries, improve patient outcomes, and enhance access to healthcare within primary care settings.

Qualifications

• GPhC registered with current status.
• CPPE Primary Care pathway completed or near completion.
• 12+ months experience in primary care/general practice.
• DBS check (update service) completed.

Responsibilities

• Act within professional boundaries, supporting and working with a team of pharmacists and GP colleagues.
• Conduct polypharmacy reviews and SMRs.
• Perform face-to-face medication reviews.
• Utilise skill mix to optimise patient care.
• Improve access to healthcare for patients.
• Support practice staff with prescription and medication queries.
• Medicine reconciliation and post-discharge reviews.
• Conduct medical audits and drive service improvement with the practice.
• Provide expert medication advice to clinicians and patients.
• Lead medicines optimisation and quality improvement initiatives.
• Ensure safe and efficient use of medicines.
• Educate patients on medications and disease prevention.
• Understand QOF, CQC and local incentive workstreams.
• Create and follow a clinical pharmacy programme aligned with practice policy.
