• Monitor, manage and adjust treatments for chronic illnesses as part of long-term condition care.
• Handle medication requests, reauthorise repeat prescriptions and address acute queries.
• Reconcile medications for patients recently discharged from hospital to reduce discrepancies and adverse effects.
• Act as the primary expert contact for medicine-related questions from Doctors, Nurses, patients and local pharmacies, supporting quality targets and cost optimisation.
