Job summary

Neston Medical Centre, part of the Neston and Willaston Primary Care Network (PCN), is seeking a Clinical Pharmacist to strengthen medicines optimisation across three practices and deliver patient-centred pharmaceutical care.

You will conduct structured medication reviews, support prescribing quality schemes, and work with GPs and other professionals to improve outcomes and reduce medication-related harm.

Qualifications

• Master of Pharmacy Degree (MPharm) and GPhC registration are essential.
• Independent Prescriber qualification or progress toward it is desirable.
• Completion of the Primary Care Pharmacy Education Pathway (PCPEP) is desirable.

Responsibilities

• Lead structured medication reviews and medicines optimisation.
• Reconcile medicines after hospital discharge and support care homes.
• Provide evidence-based prescribing advice to clinicians and patients.
• Support adherence and patient education for long-term conditions.
