Job summary

The Clinical Pharmacist in General Practice will work within a multidisciplinary team to optimise medicines, lead on SMR reviews, and support GP workload reduction. You will provide primary medicines advice, support repeat prescribing, conduct medicines reconciliation on transfers, and drive medicines optimisation and quality improvement across the Practice in Nottingham.

Responsibilities

• Lead medicines optimisation and quality improvement within General Practice.
• Undertake clinical medication reviews, including SMR, and monitor eRD prescribing.
• Provide primary medicines advice and respond to prescription and medication queries from GP staff.
• Support repeat prescribing systems, acute prescription requests and medicines reconciliation on transfer of care.
• Collaborate with community and hospital pharmacists to optimise care and manage polypharmacy.
