Job description

The Deben Health Group PCN is looking for a clinical pharmacist to join our well-established team on a full or part-time basis, with a minimum of 15 hours per week required. As a clinical pharmacist, you will join an established team with clinical supervision from within the PCN. You will be primarily based at Riduna Park in Melton, Woodbridge, but may be required to travel to other practices within the PCN.

You will provide patient-facing and remote medicines support through structured medication reviews (SMRs), medicine optimisation, GP tasks, MHRA, discharge summaries, long-term condition management, telephone medicines support to patients and GP practice staff, management of medicines after hospital discharge, and ensuring safe repeat prescribing.

Main duties of the job
  1. Work with the MDT to achieve targets set in the Direct Enhanced Services.
  2. Review patients with single or multiple medical problems requiring medicine optimisation (e.g., diabetes, asthma). This includes reviewing ongoing medicine needs, monitoring requirements, and supporting patients in medication use.
  3. Respond to medicine-related enquiries from GPs, practice staff, healthcare teams, and patients, providing solutions and recommendations.
  4. Conduct clinical medication reviews with patients (including housebound patients) and carers, producing prescribing and monitoring recommendations.
  5. Provide telephone and written advice to patients regarding their medicines, including online queries.
  6. Signpost to community pharmacy and refer to GPs or other healthcare professionals as appropriate.
  7. Collaborate with PCN registered Care Homes and Residential Care Homes to review specific medication issues.
  8. Answer queries regarding local policies, procedures, training, and vaccinations.
  9. Identify high-risk patient cohorts through practice computer searches.
  10. Analyze, interpret, and present PCN medicines data to highlight issues and support decision-making.
