Mid Norfolk Primary Care Network (PCN) Clinical Pharmacist Role

Mid Norfolk Primary Care Network (PCN) has an opportunity for an experienced clinical pharmacist, on a full-time basis, to join their growing team supporting six GP practices across the Dereham area.

Main Responsibilities
  1. Manage the prescription request workflow.
  2. Conduct complex face-to-face medication reviews, focusing on patients with polypharmacy, deprescribing, and opiate prescribing.
  3. Manage network quality activities such as sharing best practices and cross-network audits.
  4. Develop and oversee medicines optimisation services within the network practices.
  5. Lead quality improvement initiatives and clinical audits, and manage aspects of medicines safety and antibiotic stewardship.
  6. Support the management of patients with long-term conditions.
  7. Undertake clinical medication reviews with patients and provide recommendations to senior clinical pharmacists, nurses, or GPs on prescribing and monitoring.
  8. Collaborate with care home staff to enhance the safety of medicine ordering and administration.

Candidate Requirements

You will be a qualified pharmacist with significant experience in primary care. Essential qualifications include:

  • Minimum of 2 years (or equivalent) post-qualifying experience as a pharmacist.
  • Completion of an undergraduate degree in pharmacy.
  • Working towards an independent prescribing qualification.
  • Excellent interpersonal, influencing, and negotiating skills.
  • Strong written and verbal communication skills.
  • Ability to communicate complex and sensitive information clearly to diverse patients.
  • Deep therapeutic and clinical knowledge, with an understanding of evidence-based healthcare principles.
  • Excellent IT skills.
  • Ability to plan, manage, monitor, advise, and review medicines optimisation issues for long-term conditions.
  • Ability to work effectively independently and as part of a team.

Desirable qualifications include holding an independent prescribing qualification and a Clinical Diploma.
