Leaders in Care is recruiting a Complex Care Clinical Lead for Burton upon Trent to oversee delivery of high-quality, personalised care in the community. The role offers clinical leadership, mentorship, and the chance to drive improved outcomes for adults with complex health needs.

You will lead care teams, develop tailored plans, and work with CHC nurses and commissioners to coordinate effective packages while upholding exceptional care standards.

Competenze

• Registered Nurse with active NMC PIN.
• Experience delivering complex community care.
• Competent in tracheostomy and ventilator management.
• Good knowledge of CQC regulations and clinical governance.

• Develop, implement, and review personalised care plans for adults with complex needs.

As a Complex Care Clinical Lead, you will play a key role in overseeing the delivery of high-quality, personalised care packages within the community. You will provide clinical leadership, mentor and develop care teams, and help improve outcomes for adults living with complex health needs. You will be joining a growing, highly regarded provider committed to delivering exceptional standards of care.

As a Complex Care Clinical Lead you will:

• Develop, implement, and review personalised care plans for adults with complex clinical needs
• Provide clinical leadership, training, and ongoing support to care staff
• Apply specialist clinical skills, including tracheostomy care, ventilator management, and seizure management
• Collaborate with CHC nurses, commissioners, and the wider multidisciplinary team to coordinate effective care packages
• Establish positive relationships with service users and their families, ensuring care is tailored to their individual needs

Criteria to apply:

• Registered Nurse (RGN) with an active NMC PIN
• Competent in tracheostomy and ventilator care
• Full UK driving licence
• Previous experience within community complex care
• Good understanding of CQC regulations, clinical governance, and care quality standards
