Leaders in Care is seeking a Complex Care Clinical Lead in Lincoln. You will lead delivery of high-quality, personalised care packages in the community, mentor care teams, and drive clinical excellence.

Responsibilities

• Apply specialist clinical skills in tracheostomy, ventilation, and seizure management.
• Collaborate with CHC nurses, commissioners, and the multidisciplinary team to coordinate care packages.
• Establish positive relationships with service users and families to tailor care.

As a Complex Care Clinical Lead, you will play a key role in overseeing the delivery of high-quality, personalised care packages within the community. You will provide clinical leadership, mentor and develop care teams, and help improve outcomes for adults living with complex health needs. As a Complex Care Clinical Lead you will:
