Clinical Hub Team Lead (Clinical Hub Services)

Salary: Agenda for Change Band 7 £54,320 - £60,981 dependent on experience (includes HCA Supplement) plus Team Lead Supplementary

Hours: Full time (37.5 hours per week)

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of the Team Lead within our Clinical Hub Services.

This post is suited to a senior nurse with significant experience of caring for patients with life-limiting illnesses who possesses advanced knowledge and skills in clinical assessment, symptom management, and communication.

You will be a key member of the Clinical Hub Services and the wider multidisciplinary team working across Royal Trinity Hospices' community catchment area supporting patients with urgent and complex specialist palliative care needs wherever they call home.

Main duties of the job:

  1. Work closely with other Clinical/Non-Clinical Team Leads, Heads of Services, and the Director of Patient Services to provide visible and effective leadership and management support to all staff, aligning with service goals and Trinity's vision and values.
  2. Provide clinical support to members of the Clinical Hub services.
  3. Establish and maintain relationships with external stakeholders and represent the organization professionally and collaboratively.
  4. Develop and maintain organizational systems and processes that support high-quality, integrated patient care, teamwork, and communication.

Job responsibilities:

  • Responsible for all aspects of day-to-day operational leadership and management for all staff within the Clinical Hub service.
  • Lead efforts to ensure services meet local population needs, are accessible, and have clear patient pathways and processes.

The Role will include:

  • Providing visible leadership and management support alongside other Clinical/Non-Clinical Team Leads, Heads of Services, and the Director of Patient Services.
  • Providing clinical support to Clinical Hub members.
  • Establishing and maintaining relationships with external stakeholders and representing the organization professionally.

  • Overseeing the day-to-day running of the Specialist Community Nursing service in the absence of the Community Team Lead.
  • Providing clinical nursing support to the Virtual Hospice Ward.

Person Specification:

Qualifications:
  • Professional qualification and membership of the nursing profession.
  • Registered with the NMC.
Desirable:
  • Mentorship and teaching qualification.
  • Postgraduate learning in Palliative Care or working towards it.
Experience:
  • Leadership and management experience.
