Job summary

Horder Healthcare is seeking an experienced Clinical Coder to ensure accurate, timely clinical information across our organisation. You will translate diagnoses and procedures into ICD-10 and OPCS-4 codes, in line with national standards, and work with consultants, nursing and administrative teams to support patient care and financial reporting.

You will review case notes, discharge summaries and other clinical documents, maintain HRG accuracy, and contribute to audits and policy improvements

Qualifications

• Knowledge of ICD-10 and OPCS-4 coding rules.
• Experience in coding within a clinical environment.
• Completion of the CCSC or equivalent certification.
• Proven accuracy in coding diagnoses, co-morbidities and procedures.
• Ability to meet deadlines and work under pressure.
• Strong communication with clinical and administrative teams.

Responsibilities

• Review case notes, discharge summaries and clinical docs to code ICD-10/OPCS-4.
• Code diagnoses, co-morbidities and procedures with accuracy.
• Meet monthly coding deadlines and timescales.
• Maintain HRG awareness and support financial reporting.
• Assist audits and improve clinical documentation quality.
• Provide guidance on coding to clinical and admin staff.
• Keep up to date with ICD-10, OPCS-4 changes and standards.
• Contribute to coding policies and process improvements.
• Deliver training to colleagues on coding importance.
• Collaborate with IT to keep coding systems updated.
