Leaders In Care Recruitment Ltd in Bournemouth is seeking an experienced Care Coordinator to own rotas, support care teams and deliver high-quality community care. The role is Monday to Friday, 9am–5pm, with additional on-call duties on a rota basis.

You will coordinate visits, supervise care workers, review care plans, and build strong relationships with clients and families. A full UK driving licence and access to your own vehicle are essential.

• Strong organisational and communication skills are essential.

Are you an experienced Care Coordinator looking for a role where you can take ownership of your rotas, support your care teams and play a key part in delivering high-quality care within the community? We are looking for a Care Coordinator to join an established homecare provider in Bournemouth. You'll work Monday to Friday, 9am–5pm, alongside additional on-call duties on a rota basis.

As a Care Coordinator , you'll be responsible for:

• Supporting and supervising care workers in the community
• Assessing and reviewing individual care plans
• Completing field-based visits with clients and care staff
• Building strong relationships with clients, families and care workers
• Responding to changes in client needs and resolving day-to-day issues
• Ensuring care is delivered safely and to a high standard

You'll need previous experience within care coordination or a similar community care role, with strong organisational and communication skills and the ability to manage busy rotas effectively.

A full UK driving licence and access to your own vehicle are essential.
