Job summary

CQ Medicentre South in Rockhampton is seeking an experienced Registered or Enrolled Nurse to take a lead role in Chronic Condition Management, proactive care planning and patient education within a supportive GP team.

The full‑time position focuses on coordinating management plans, conducting health assessments, and follow‑up, while collaborating with GPs and allied health staff to improve long‑term patient outcomes and maintain comprehensive clinical documentation.

Qualifications

• Must have current AHPRA registration as a Registered or Enrolled Nurse.
• CPR certification is required.
• Experience in General Practice nursing.
• Demonstrated experience with Chronic Condition Management Plans, Care Plan Reviews, Health Assessments and patient follow‑up programs.
• Excellent organisational and communication skills.
• Full Australian working rights.
• Experience using Best Practice software will be highly regarded.
