Job summary

City Heart Medical in Rockhampton is seeking a full-time Registered Nurse to work with our dedicated team as a Chronic Disease Management Nurse. You will provide high-quality patient care with a focus on care plans, reviews and comprehensive health assessments.

You will work both autonomously and collaboratively to deliver excellent nursing support, patient education and follow-up, while holding AHPRA registration and CPR certification; Australian working rights are required.

Qualifications

• You're a nurse with AHPRA registration and full working rights in Australia (no sponsorship available).
• You're experienced in GP Chronic Condition Management Plans and Health Assessments.
• You’re passionate about patient care.
• You hold a current CPR certificate - although assistance can be offered here if it has recently expired.

Responsibilities

• Deliver general practice nursing with a strong focus on care plans and reviews, health assessments, patient education and follow-up.
• Work autonomously and collaboratively to provide high-quality patient care.
