Job summary

CNTW NHS Foundation Trust is seeking a Band 5 Community Nurse to join the Children’s Community Learning Disability and Behaviour Support Service in North Cumbria. You will support children with learning disabilities and/or autism and their families, delivering high-quality, sustainable care within the Positive Behaviour Support framework.

Qualifications

• Experience of working with service users and carers in successful delivery of treatment plans
• Complete preceptorship programme for newly qualified nurses
• Multi-disciplinary team work
• Proven ability to develop therapeutic partnership with service users and carers
• Proven ability to communicate interventions to service users
• Further relevant post qualifying training
• Previous experience in a community setting
• Experience of involving service users/ carers in evaluation/planning and monitoring of services
• Experience of working with children and young people

Responsibilities

• Work within the framework of Positive Behaviour Support to increase quality of life for children and families.
• Deliver proactive support for children with learning disabilities or Autism and challenging behaviours to reduce tier 3 service use.
• Facilitate parent programmes alongside managing a caseload.
• Work autonomously with guidance from the multidisciplinary team.
• Travel to community premises and sites as needed to meet service requirements.
