Job summary

West Northamptonshire Council is seeking an Assistant Team Manager for Continuing Healthcare to supervise social workers and practitioners, guiding high-quality support for adults with complex health and social care needs. You will work with the Team Manager to promote best practice and positive outcomes, supporting development and service improvements.

The role focuses on leadership, supervision, and cross-agency collaboration with NHS colleagues to ensure person-centred care and robust

Qualifications

• Qualified Social Worker registered with Social Work England.
• Experience in adult social care and CHC is preferred.
• Experience supervising and guiding practitioners.

Responsibilities

• Providing supervision, coaching and professional support to team members.
• Offering advice and guidance on complex cases, safeguarding concerns and risk management.
• Leading on quality assurance activity, audits and practice development.
• Supporting recruitment, induction and development of colleagues.
• Contributing to service improvements and helping shape the CHC service.
