Job summary

West Sussex County Council in Horsham is seeking a self-motivated assistant care manager to join our Lifelong Services Team. This part-time role (30 hours per week) is based at Horsham office with hybrid working and permanent contract.

Under guidance and supervision you will provide social work support including assessment, planning and review within the Supporting Lives, Connecting People framework, managing a challenging portfolio of casework.

Qualifications

• GCSE English and Maths at minimum literacy level.
• NVQ Level 3 in Care or equivalent experience.
• Experience in providing support in a Social Care environment.
• Experience forming effective relationships in a multi-agency setting.

Responsibilities

• Provide social work support including assessment, planning and review.
• Manage a challenging portfolio of casework with supervision.
• Attend multi-disciplinary meetings and collaborate with Health, Community and Third Sector colleagues.
