Livewell Southwest is seeking a Band 5 Continuing Health Care practitioner for a 12-month fixed-term maternity cover within Mental Health Services. The post holder will assess and review care needs for health-funded individuals, developing outcome-based support plans.

The role is 0.4 FTE with a strengths-based, solution-focused approach and seven-day service availability. Experience in health or social care and a relevant NVQ level 3, plus knowledge of Care Act 2014, are essential.
