The post holder will be responsible and accountable for the delivery of an accessible, appropriate, high quality and effective nursing service to all age groups in the community by working in partnership with patients, care providers and other agencies.
The post holder will ensure a person-centered approach to care delivery that is safe and effective, maintaining quality standards.
The post holder will function as an independent autonomous practitioner with in-depth knowledge, expertise, and experience to lead and enable the Community nursing team to provide quality nursing services.
The post holder will lead, manage, and support the Community nursing team, be a clinical expert, involved in public health activities, accountable for a caseload over 24 hours, and be a key member of the Integrated Primary Care team and other multidisciplinary groups.

Permanent, Full-time or Part-time, 37 hours/week
Based at ICST Community Nursing, Hunter Health Centre. Working hours: 0745-1545, 0845-1645, 1230-2030.
