About the role
To provide early and targeted support to aid the recovery and resilience of people with multiple vulnerabilities, working across a portfolio of longer-term accommodation-based services intended to positively address the ‘revolving door’ nature of shorter term supported accommodation. Demonstrating a commitment to safeguarding in line with policy and procedure, you will support customers to develop the knowledge and skills they need to achieve their goals, empowering them to build successful relationships within their community and access the necessary services to develop skills to better manage their health and wellbeing and increase levels of independence as they move through a prevention pathway model.
