About the role

Due to continual growth within our support services, we are currently looking to recruit a Specialist Mental Health Support Worker based at our supported living rehabilitation service in Shropshire. The role will be based in Shrewsbury to provide rehabilitation-focused support to individuals with complex and enduring mental health needs, working as part of a 24-hour service. Demonstrating a commitment to safeguarding in line with policy and procedure, you will support customers with life skills and empower them to build successful relationships within their community, as they develop skills to better manage their health and well-being and increase levels of independence within their daily living and tenancy management.

Key Responsibilities

  • Deliver and promote a high standard of support for each individual in the home, including demonstrating and advising on good practice.
  • Promote individual customers’ independence and choice.
  • Prompt individual customers with all aspects of personal hygiene and appearance as required by the individual.
  • Advise and assist in developing a range of social, leisure, work, education, or other meaningful activities as appropriate with customers. This may involve supporting customers to use ICT to access information and services.
  • Support customers in preparing and cooking meals, including menu planning and budgeting.
  • Support customers to take an active role within the community.
  • Assist customers in preparations to move on from services, liaising with care coordinators, social workers, and other care providers.
  • Establish and maintain links with other healthcare professionals and agencies.
  • Undertake the key-working role as designated by your line manager, including regular key-work sessions, reviewing support plans, and participating in review meetings with the customer.
  • Undertake initial and ongoing assessments in consultation with the customer, manager, and involved agencies.
  • Promote and uphold the Trident Reach policy on Equality and Diversity.
  • Maintain the confidentiality of all customers and staff information.
  • Adhere to the Trident Reach Policies & Procedures and Code of Conduct.

Staff can expect regular supervisions from their line manager; they should be available and prepared with their agenda in advance of the meeting.

About You

We are looking for a highly motivated individual who would like to join our team to deliver good quality service to vulnerable adults.

  • Able to work flexibly within a 24-hour rota system, including evenings, weekends, and sleep-in shifts.
  • Experience working with customers with complex mental health needs, including self-harm, personality disorder, and schizophrenia.
  • Self-motivated and able to work on own initiative.
  • Experience of partnership working.
  • Deliver customer-focused support in line with customer hours and service rotas.
  • Ability to create and maintain personalized support plans.
  • Assess risks, write assessments, and develop risk management plans.
  • Hold regular key worker meetings to review and update customer outcomes.
  • Ability to use Microsoft IT programs.
  • NVQ Level 2 in Health and Social Care.
  • Experience in supported housing settings.
