Job summary

Single Homeless Project in London is seeking a Project Worker to manage a caseload, build trusting relationships, and deliver person-centred assessments and safety plans to people facing homelessness and complex needs.

You will work with health, social care, housing and criminal justice partners to coordinate joined-up support, maintain accurate records, and help remove barriers to accessing services, with training and development opportunities available.

Qualifications

• Ability to meet people where they are and build trust with those facing homelessness and complex needs.
• Patience, creativity and warmth in approach.
• Capable of managing a caseload while focusing on the person behind the plan.
• Willing to collaborate with health, social care, housing and criminal justice partners.
• Motivated by helping people build confidence, independence and stability.

Responsibilities

• Hold a caseload and build purposeful relationships with clients.
• Complete assessments, develop person-centred support and safety plans.
• Advocate for clients and coordinate joined-up support.
• Maintain clear and accurate records and contribute to the safe running of accommodation.
• Respond calmly to risk and safeguarding concerns.
