About the role

We are looking for an organised, proactive and customer-focused Case Support Co-ordinator to join our growing Business Support Hub for a 12-month fixed term contract.

This is a full time role to support our Adult Social Care team in Purbeck, based in Wareham.

As a Case Support Co-ordinator, you'll play a vital role in helping social care practitioners deliver support to adults and older people across Dorset.

Working closely with Social Workers, Occupational Therapists, Carers Case Workers and Assessment and Support Co-ordinators, you'll help ensure people receive timely, effective support by providing high-quality administrative and business support.

No two days are the same, and you'll be part of a supportive team that plays an essential role in helping frontline services make a difference to people's lives.

What You Will Be Doing

You will provide a wide range of administrative and coordination support, including:

  • Coordinating and processing referrals into Adult Social Care services.
  • Managing appointments, meetings and diaries for practitioners and teams.
  • Liaising with customers, families, health professionals, commissioned providers and partner organisations.
  • Handling telephone and email enquiries, resolving routine queries and signposting where appropriate.
  • Preparing correspondence, reports and documentation.
  • Maintaining accurate records and ensuring information is entered correctly onto our case management systems.
  • Uploading, scanning and processing documents in a timely and accurate manner.
  • Supporting statutory and multi-agency processes through the preparation of paperwork and documentation.

You will also be responsible for producing meeting write-ups from recordings of meetings, including:

  • Best Interest Meetings
  • Multi-Agency Risk Management Meetings
  • Other professional meetings as required
About You

We're looking for someone who:

  • Has experience working in an administrative, office or customer service environment.
  • Is highly organised and able to manage competing priorities.
  • Has strong communication and interpersonal skills.
  • Confident using Microsoft Office applications, including Outlook, Teams, Word and Excel.
  • Excellent attention to detail and can maintain accurate records.
  • Enjoys working as part of a team while being able to manage their own workload.
  • Are flexible, proactive and committed to providing excellent customer service.

Experience of working in a social care, health or public sector environment would be beneficial but is not essential.

Working Arrangements

This is a hybrid role. However, successful candidates should expect to spend a significant amount of time working from an office location during their initial induction and training period to support learning, development and integration into the team. Following this, a hybrid working arrangement will be discussed with your manager in line with service needs and Dorset Council's hybrid working arrangement, but there would be an expectation of at least two days working from the Wareham office.
