Job summary

Arizona Department of Economic Security is seeking a Case Manager 3 to support individuals with developmental disabilities and their families. Responsibilities include developing person-centered service plans, coordinating services, and collaborating with community agencies to assist families in crisis.

The position requires an AZ RN license or a Bachelor's in Social Work/Psychology/Special Education/Counseling with 1+ year of case management experience, or 2+ years with broader disability/SMI

Qualifications

• Must have AZ RN license OR a Bachelor's Degree in Social Work, Psychology, Special Education or Counseling with at least 1 year of case management experience.
• Or at least 2 years of case management experience for elderly, disability, or SA/MH disorders.
• Must have a valid driver’s license and pass background checks; E-Verify.
• Fingerprint clearance card as required and pass registry checks.

Responsibilities

• Drive to members' homes, schools, or program sites to discuss service needs.
• Develop, review, monitor, and evaluate Person-Centered Service Plans (PCSP).
• Facilitate meetings and visits to assist with member/family choices; complete referrals.
• Collaborate with community agencies to provide support to members and families in crisis.
• Write progress notes, maintain records, and prepare reports.
