Job summary

Carers Trust Crossroads West Wales in Haverfordwest seeks a Carers Wellbeing Officer for a fixed-term post until 30 September 2027. The role is 21 hours per week with a salary of £24,920 per annum pro rata, based in Pembrokeshire.

You will work in partnership with statutory, voluntary and private sector organisations to identify carers and provide information and support to enhance their wellbeing and access services.

Qualifications

• Direct experience of working for a support service and achieving positive outcomes for beneficiaries
• Experience of networking and liaising with a range of statutory and voluntary organisations
• Experience of supporting unpaid carers
• Excellent communication and interpersonal skills
• Ability to plan, prioritise and manage workload

Responsibilities

• Identify new carers within Pembrokeshire and actively support uptake of services
• Provide information, support and outreach to enable carers to manage health, financial and social impacts
• Facilitate a What Matters to Me conversation to assess needs and outcomes with carers
• Signpost and refer carers to other agencies and services as appropriate
• Support carers to develop contingency plans and access grants where eligible
