About the Role

Care Workers at Level 2 implement Care Plans, report concerns or issues that prevent adherence to these plans, and monitor the care and welfare of citizens. They record accurate information on running records, communication books, care plans, and monitoring charts (including weight, falls, and other relevant data).

Level 2 Care Workers collaborate with other professionals, citizens, and their families to ensure a shared approach to care, support, and continuity of treatment.

The role involves providing safe, appropriate, and personalized care that maintains dignity, maximizes independence, and empowers citizens with choice and control. Responsibilities include writing and maintaining individual person-centered care plans and administering medication.

The position may involve working in various areas of Adult Social Care, including in-home, community settings, or within a Council establishment, and with citizens having diverse needs.

About You

The ideal candidate will have:

  • The ability to support citizens in exercising choice and control based on their needs, preferences, and aspirations.
  • The ability to encourage, motivate, and support citizens to maintain or regain independence in daily activities.
  • Experience in developing and coordinating leisure and social activities that promote self-esteem, confidence, and purposeful engagement, including one-to-one conversations, group activities, and supporting citizens with their interests, family contacts, outings, shopping, pensions, etc.
  • The ability to write and maintain up-to-date, individual, person-centered care plans.

A DBS enhanced check is required for this regulated activity.
