Job summary

Candlelight Care in Glastonbury is seeking a proactive Team Leader to oversee client packages, develop care plans, carry out risk assessments and reviews, and support our team of Care & Support Workers. You will liaise with the Registered Manager and clients and contribute to care progress.

The role includes staff observations, on-call responsibilities, and attending Team Leader meetings. A full driving licence and own transport are required; alternate weekends and evening shifts are part of the

Qualifications

• Diploma level 2 in Health & Social Care or equivalent.
• Full (clean) driving licence and access to own transport.
• Available to work alternate weekends.
• Be part of the on-call rota team.
• Good communication skills.
• Ability to work without supervision.
• Caring and empathetic approach.

Responsibilities

• Evaluate and assess new client packages and develop care plans.
• Carry out risk assessments and reviews of existing packages.
• Liaise with the Registered Manager and clients; monitor care progress.
• Undertake staff observations and supervisions.
• Be part of the on-call team on a rotational basis.
• Attend Team Leader meetings.
